You can now apply for LaMOMS online with our easy-to-use online Medicaid application. You can use the online Medicaid application to apply for all Medicaid programs including LaMOMS! Your online applications will be processed faster because once you click ‘submit’ it goes immediately to the local office where your application will be processed. If you prefer to apply for LaMOMS by filling out a paper application form, you can download the form listed below, print it and then mail or fax it to us.
