You can now apply or renew LaCHIP and LaCHIP Affordable Plan online with our easy-to-use online Medicaid application.

You can use the online Medicaid application to apply or renew all Medicaid programs including LaCHIP and LaCHIP Affordable Plan!  Your online applications will be processed faster because once you click ‘submit’ it goes immediately to the local office where your application will be processed. If you prefer to apply for LaCHIP or LaCHIP Affordable Plan by filling out a paper application form, you can download the form listed below, print it and then mail or fax it to us.
