OTIS Welcome Page
Welcome to OTIS
OTIS (Online Tracking Incident System) is a web based application created by DHH. Nursing homes or ICF/DD providers submit via the Internet State and Federally required reports of alleged neglect, abuse, injuries of unknown origin, and/or misappropriation of resident property. Injuries of unknown origin are defined as those injuries which meet the following two conditions:
1) the source of the injury was not observed by any person or the source of the injury could not be explained by the resident AND
2) the injury is suspicious because of the extent of the injury or the location of the injury (e.g., the injury is located in an area not generally vulnerable to trauma).
All injuries of unknown origin not meeting the two conditions noted above are required to be investigated by the facility and documentation of their investigation is maintained in the facility.

OTIS Passwords - Each person that enters incidents in OTIS must have their own username and password. It is a violation of HIPPA policy to login to OTIS under another person's username.
Requests for usernames and passwords should be on company letterhead and signed by the facility administrator. Please fax requests to the appropriate person listed under "Contact Information" listed at the bottom of this page. Passwords will be mailed the administrator within 5 work days via certified mail.
OTIS Password Rules

Passwords will expire every 180 days.

New password must be six characters including one numeric character. (No special characters such as *, &, /, etc.)

cannot reuse last 3 passwords

User will be locked out after 3 incorrect attempts to login.

OTIS user accounts will be automatically disabled after 180 days of inactivity.

view previously submitted reports for your facility; use same username and password as used to enter an OTIS incident.
2. OTIS Client - Accused Reports - search by client (victim) or accused name or SSN
OTIS Training Web Site - enter test data only; must have a username and password issued by HSS to login
