
Home Based Care Program
What is the Home Based Care Program?
The Home Based Care Program, which has been under the administration of HAP since 1992, provides assistance to HIV-infected individuals by providing in-home health care and personal care attendant services.
The program is designed to provide gap coverage when other types of coverage expire or are inadequate, or when a client has no other types of coverage available for home health services. Eligible clients do not need to be homebound to qualify for Ryan White Home Based Care or Hospice services.
What services are provided through the Home Based Care Program?
The program includes services such as skilled nursing or home health aide visits and the provision of supplies and durable medical equipment. Other services available through this program include physical therapy and social work services. Personal care services are also available to provide assistance in daily household activities for people who are homebound.
Who is eligible for the Home Based Care Program?
In order to be eligible for the Home Based Care Program, individuals must be HIV-infected and have a physician's orders for medical care which can be provided in a home setting.
Other requirements include that the participant must have:

An annual household income of less than 200% of the federal poverty guidelines

Available resources of less than $4,000, excluding a house and/or car

A physician who is willing to act as his/her primary care provider while home based care services are provided
