Welcome to the Office of Aging and Adult Services (OAAS) OTIS Home Page

W-OTIS Overview                                                              

The Online Tracking Incident System (OTIS) is a web-based application initially developed by the Louisiana Department of Health and Hospitals (DHH) in April of 2003, with the primary purpose of providing them with an optional method (via the internet) for submitting State and Federally required reports of alleged neglect, abuse, injuries of unknown origin, and/or misappropriation of Resident property.  All DHH patient care and residential care facilities were also provided with access to OTIS in April of 2003, and are required to use OTIS for reporting and tracking of all abuse and neglect incidents.  

The DHH recently released a community version of OTIS (W-OTIS).  As a result of this expansion to the OTIS application, Home and Community-Based (HCBS) oversight agencies and Support Coordination agencies (SCA) will now be able to electronically submit, follow-up, track and trend critical incidents involving persons receiving services in the community, in accordance with applicable DHH Critical Incident Reporting policies and procedures.

W-OTIS System Security and Password Assignment

Security built into OTIS allows access to only those authorized to access the system. Each person that enters incidents into OTIS, must have their own username and password.  It is a violation of HIPPA policy to login to OTIS-HCBS under another person's username. It is the responsibility of each SC agency to notify the OAAS Regional office of all separated and newly hired employees.  The OAAS Regional office performs password maintenance for SC’s serving OAAS participants.  The Office for Citizens with Developmental Disabilities (OCDD) performs password maintenance for SC’s serving OCDD participants.  Only one password will be issued for each SC even if they serve both populations. Passwords will not be issued to SC agency employees who are not SC’s.
