Louisiana ADAP Recertification
Most LA ADAP Cards expired on June 30th
*To continue receiving LA ADAP services recertification forms must be complete and fax along with all required documentation to 504-568-7042.
* There is a grace period throughout July in which 08/09 ADAP cards with Gold seals CAN be used to access refills. However, all LA ADAP services will be discontinued for individuals who do not complete the recertification process prior to August 1, 2009.
* Contact your case manager, social worker or LA ADAP at 504-568-5489 or for more information.
