Facility Closure Procedure

Thirty (30) days prior to closure of the facility ADHC provider shall submit a Letter of Intent to the ADHC Program Desk. The closure letter shall state:

reason for closure
date of closure
how the facility records will be retained

Following closure of the facility the provider shall return the original license to Health Standards Section (HSS).
