Adult Day Health Care License Renewal

The following is required to complete the Adult Day Health Care renewal process:

1. Adult Day Health Care Provider Renewal Application. 
2. Application license fee
3. $600.00 license renewal fee
4. Current State Fire Marshall Report.  Facilities located in New Orleans must also submit the city Fire Marshall Report
5. Current Office of Public Health inspection report

Please be sure to complete all sections of the renewal application.  If a particular section does not apply to the facility please insert N/A.  If all sections of the application are not completed, the application will be returned to the administrator.
