Adult Residential License Renewal Procedure

Please be sure to complete all sections of the renewal application.  If a particular section does not apply to the facility please insert N/A.  If all sections of the application are not completed, the application will be returned to the administrator. 

 

  1. Adult Residential Care Provider Renewal Application form   
  2. Adult Residential Care Provider Waiver Questionnaire
  3. Applicable license renewal fee
    * Any ARCP previously licensed by DSS as of June 30, 2010 shall pay a $250 license renewal fee.
    * Any ARCP previously licensed by DSS as of June 30, 2010 that changes their level, shall pay $600  plus $5 per unit
  4. Current State Fire Marshall Report.  Facilities located in New Orleans must also submit the city Fire Marshall Report
  5. Current Office of Public Health inspection report
  6. Current proof of current general and professional liability insurance of at least $300,000 each
