Chronic Disease Database in Louisiana Report

The Department of Health and Hospitals (DHH) submits the attached report in response to HR 138 of the 2012 Regular Session. HR 138 requires DHH to submit a report to the House of Representatives regarding the feasibility of establishing a chronic disease database in Louisiana on Chronic Care Management in the Community.
