Providers who desire initial licensure to operate a Home and Community Based Service (HCBS) should review the following documents:

- Facility Need Review Rule -09/2011 - Explains application process & Review Period
- Facility Need Review Rule - 03/2012 - fee changes
- submit an application & fee(s) for Facility Need Review (FNR)
