Supportive documents, such as statements, copies of checks, copies of bank statements, etc., may not be uploaded into the OTIS system. Please record the OTIS Incident ID number on supportive documentation and retain in your files. Should HSS determine additional information is required, a request will be made to the facility.
Required Information - If Accused is a staff person, then accused SSN, Title and Address are required.
