Hospital Licensing Standards Emergency Preparedness - Electronic Reporting Requirements (LAC 48:I.9335). La. Register Vol 35, No. 02, Feburary 20, 2009.

REQUEST FOR EMERGENCY RESOURCES SHOULD BE ROUTED THROUGH YOUR PLANNED CHANNELS STARTING WITH LOCAL EMERGENCY RESOURCES!

Facility Status – To be updated as requested

Operating Status- Which parts of hospital are: Open, Closed, Date, Notes; Evacuation Status- Fully evacuated, Partially evacuated, Sheltering In Place (SIP); Quick Update Box- for use only if all facility status information is unchanged from last update; Fuel Status- adequate, critically low, out, date.

Facility Contacts – To be kept current, update as information changes

Emergency Contacts– Names, Types/Positions, Phone and Email information; Utility Providers- Name, account #, Type: Electricity, Water, Natural Gas providers; Evacuation Host Sites and Emergency Transportation Providers- Name, Addresses, Contact info.

Bed Types – To be kept current, update as information changes

Hospitals need to choose the types of beds they have and staff. The hospital will then need to report census information on these bed types.

Facility Census for Hospitals shall be updated as required or requested

Bed Types need to be entered first then for each bed type enter Staffed Capacity; Census; Available Beds, Available in 24 hours; Available in 72 hours.

Facility Generators – To be kept current, update as information changes

Generators- Make/Model, Output in Kilowatts, Phase (single or three), Voltage, Burn rate (fuel consumed in one hour) for each generator matched to its tank; Generator Services Powered- what does each generator supply power to(Ex.- Elevators, Food refrigeration\Kitchen, Full HVAC, General Lighting, Partial HVAC, Pharmacy, etc); Fuel Tanks- list of fuel tanks. Match the fuel tank(s) to its generator(s) , provide location of tank for refueling.

Encounters – To be kept current, update as information changes

HHS Questions - H1N1 Report- answer each question

Answer each question, use drop down when available. Add notes as necessary.

Facility Power Outages – To be updated as requested

Incident- what event (storm, event) is the cause of the outage; Event- what resource (power, water, phone), when did it go out, when was it restored; Generator- electrical outages only – generator being used, tank being used, fuel burn rate, gallons of fuel
