Online Tracking Incident System (OTIS)

OTIS is a web-based application the Louisiana Department of Health and Hospitals (DHH) developed in April 2003. It provides state agencies, health care providers, and Home and Community Based Services (HCBS) support coordinators an online method for submitting state and federally required reports of alleged neglect, abuse, injuries of unknown origin, misappropriation of client property and critical incidents.  If you are not reporting to the Office of Aging and Adult Services (OAAS), select the appropriate agency below to access their OTIS informational page.

W-OTIS System Security and Password Assignment

Security built into OTIS allows access to only those authorized to access the system. Each person that enters incidents into OTIS, must have their own username and password.  It is a violation of HIPPA policy to login to OTIS-HCBS under another person's username. It is the responsibility of each SC agency to notify the Office of Aging and Adult Services (OAAS) regional office of all separated and newly hired employees.  The OAAS regional office performs password maintenance for SC's serving OAAS participants.  The Office for Citizens with Developmental Disabilities (OCDD) regional office performs password maintenance for SC's serving OCDD participants.  Passwords will not be issued to SC agency employees who are not SCs.

Listed under "Related Items" below are links to all current OAAS CIR policy, forms and training materials.  These links include portals to the OTIS database sites for authorized users only (support coordinators and waiver staff).  The remaining policy, forms and training materials apply to both Direct Service Providers (DSP) and Support Coordinators (SC) unless specified otherwise. The training PowerPoints include both slide and handout versions.
