The GNOCHC program is a special program called a 1115 Demonstration Waiver approved by the Centers for Medicaid and Medicare Services. It is funded through the Primary Care Access and Stabilization Grant. It is intended to preserve primary and behavioral health care access restored and expanded after Hurricane Katrina.
You must live in Orleans, Jefferson, Plaquemines, or St. Bernard parishes to qualify for GNOCHC. As long as you live in one of these parishes, you will continue to be eligible. When you move to another parish, report it to the Medicaid office at 1-888-342-6207. They will review you for other Medicaid programs.
Your application is reviewed for all Medicaid programs. Since GNOCHC doesn’t cover everything, it is one of the last programs Medicaid looks at. If you are eligible for a program with more benefits, you will be put into that program. When you are found eligible for Medicaid or GNOCHC, you will get a letter from Medicaid and a plastic health insurance card.
You are eligible for one year, unless you reach age 65, move to another parish that doesn’t have GNOCHC, you get health insurance, become pregnant, are incarcerated, or are admitted to an institution. Each year you are reviewed for continued eligibility. You may or may not be contacted. If you are found eligible again, you will receive a letter from Medicaid.
